Medication Request Form

You can order ongoing medication online using our medication request form

Please note: we are only able to dispense medication if our vets have examined your pet and reviewed the conditions for which they are receiving ongoing medication, within the last 6 months. The vet also needs to have approved more medication being dispensed at the last health check.

Please allow at least 3 week days for medication requests to be processed. If medication is needed urgently, please phone the clinic and our team will do their best to assist you.

If multiple medications are required, please complete a new form for each medication.
